Incident report
The report, and — where the state is one we have read — who has to be told.
It saves as you type, because these get written on a phone in a corridor at two in the morning. When you have said what happened, it shows what the rule asks in the 6 states whose reporting requirements we have read — who has to be told, on what form, by when, with the citation. In a state or a license class we have not read, it names the gap instead of showing an empty screen, because silence on a compliance screen reads as “nothing owed”. It will not decide for you whether something is reportable; that judgment is yours, and it says so where it matters.
What the incident report is for
Record what happened while it is fresh, and know who has to be told. Saves as you type, because these get written on a phone in a corridor at two in the morning. When you have said what happened, it works out what the rule asks of you in the states whose reporting requirements we have read — who receives it, on what form, by when, with the citation. In a state or a license class we have not read, it names the gap rather than showing an empty screen. It will not decide for you whether something is reportable. The thing you came to do is “write up what happened”, and this is the page that does it.
It is filed under compliance & safety — emergency plans, drills, training, permits and the records a surveyor asks for — and under “log something”, which is write down what happened, while it is fresh. There is one page for it, at this address, however you arrived.
Written for care and facility staff, and whoever reviews the record afterwards.
When people reach for it
- Write it up while it is fresh, and see who has to be told — people arrive here typing “incident”, “incident report”, “accident report”.
What it records, and where the record goes
The report, and — where the state is one we have read — who has to be told. Each entry is kept in this tool's own register rather than as a loose document, which is what lets the register answer the question months later instead of you re-reading a stack of paper. What you enter stays in this browser, on this device. There is no account to make and nothing is uploaded; it reaches another device only if you activate CompliMaint Cloud Beta and sign in.
- Records — every document this device holds, filed as you work.
- CompliMaint Facilities — Know what is required, do it on the day, keep the record that proves it.
- Incidents — What went wrong, who owns it, when it closed.
