Near miss report
The same record as an incident, without the duties an injury would trigger.
A near miss is the cheapest incident report you will ever read. This is the same form — what happened, who saw it, what has to change — and because nobody was hurt, the state reporting duties an injury would trigger do not appear. If the facts say somebody was hurt, it tells you to file it as an incident instead.
What the near miss report is for
Record what happened while it is fresh, and know who has to be told. A near miss is the cheapest incident report you will ever read. Same form, same investigation, same corrective actions — and because nobody was hurt, the state reporting duties an injury triggers do not appear. If the facts say somebody was hurt, it tells you to file it as an incident instead. The thing you came to do is “write down the close call”, and this is the page that does it.
It is filed under compliance & safety — emergency plans, drills, training, permits and the records a surveyor asks for — and under “log something”, which is write down what happened, while it is fresh. There is one page for it, at this address, however you arrived.
Written for anybody who saw something that could have hurt somebody.

What it records, and where the record goes
The same record as an incident, without the duties an injury would trigger. Each entry is kept in this tool's own register rather than as a loose document, which is what lets the register answer the question months later instead of you re-reading a stack of paper. What you enter stays in this browser, on this device. There is no account to make and nothing is uploaded; it reaches another device only if you activate CompliMaint Cloud Beta and sign in.

- CompliMaint Facilities — Know what is required, do it on the day, keep the record that proves it.
- Incidents — What went wrong, who owns it, when it closed.
