Almost everything lost between shifts is lost in the last five minutes of one. Four columns and a body box is enough to stop it, and it is the same four every time.

- Anything the next shift would be annoyed to discover on their own. What broke, what was reset, what is being watched, what somebody promised to call back about. Specific beats complete.
- Does completing it discharge any duty?
- No. It asserts nothing about compliance and discharges no requirement — it is what one person tells the next, written down so it survives the walk to the parking lot.
- Who signs it?
- Nobody signs on screen. The sheet names who handed over and who took over, and prints for signature on paper if that is your practice.
- Can I record something about a colleague?
- Keep it operational. This is a shift record read by everybody who comes on next, not a personnel file.
