What this form deliberately does not ask
There is no box for a Social Security number, a bank account, a date of birth or a driver’s license number, and there will not be one. A paper application often asks for all four; putting them in a browser tool with no account behind it would make this the highest-risk thing in the product. There is also no field for anything that could carry a protected characteristic — race, religion, national origin, disability, familial status, age — because a box invites somebody to fill it in, and a decision must never be traceable to one.
It screens nobody
No background check, no credit check, no score, no eligibility decision and no screening criterion. Whether you accept an application, and on what basis, is your decision and is governed by fair housing and tenant screening law where the property is. This tool takes down what somebody told you and prints it back.
The consent wording is yours, word for word
The box at the bottom is empty and stays empty until you write in it. What an applicant must be told, what they are consenting to, and what you may do with what they give you depend on your state, your process and your attorney — so CompliMaint supplies no language and no authorization. A generator that filled that box in would be writing a legal statement on your behalf.

- Why is there no place for a Social Security number?
- Because this tool is not built to hold one. Everything runs in one browser on one device with no account behind it, and a Social Security number, a bank account or a government identification number is the last thing that should live there. If your process needs them, they belong in a system built for them.
- Does it run a background or credit check?
- No. It connects to nothing and checks nothing. It is a form and a printed record; screening is a separate decision you make, subject to the screening and fair housing rules that apply where the property is.
- Does it score or rank applicants?
- No, and it will not. There is no score, no ranking and no eligibility calculation. A tool that produced one would be making a housing decision, which is not something a free form builder should be doing on anybody's behalf.
- What should go in the acknowledgment box?
- Whatever your own process and your attorney say belongs there. CompliMaint provides no consent language, no authorization wording and no privacy statement — the box is empty until you write in it, and anything left blank prints blank.
- Can an applicant fill this in themselves?
- It is built for you to take down what they tell you, in person or on the phone, and print it. Nothing is submitted anywhere: there is no form to send and no server to receive it.
