The notice sets out a provision and what you observed. It reaches no conclusion, and it is worded so that the letter does not either — the observation section asks for facts with dates rather than a finding. That distinction is not pedantry: a notice that asserts a conclusion it cannot support is the notice that comes apart when somebody disputes it.

The cure period and the consequence are both yours

Cure periods come from the lease and from state law and differ sharply. CompliMaint states none and fills nothing in. How long a cure period should I give?
CompliMaint does not say. It comes from your lease and from the law where the property is, and it differs sharply between states. The box is empty and prints what you type, attributed to you.
Does this start an eviction?
No. It is a letter. Whether a notice like this is a required step, what it must say and how it must be delivered are all set locally, and getting any of them wrong can invalidate what follows. Have a local attorney read one before you rely on it.
What should I write in the consequence section?
Only what you have decided to say, from your own lease. CompliMaint suggests nothing there and leaves it blank — a pre-written threat in a free template is the last thing anybody needs on a letter with their name on it.
Should I record the complaint separately?
Yes. The complaint register keeps what was reported and what you did; this notice is a decision to write to somebody. Keeping them apart is what stops a report turning into a finding automatically.
