A log, not a file
The register answers “did we deal with it, and when” — which is the question asked months later, usually by somebody who was not there. The substance belongs somewhere confidential, and the log says where.
Two states, and no findings
Open or closed. No category, no protected-class field, no severity, and no substantiated, unsubstantiated or upheld outcome anywhere in it.

Questions people ask
- Why is there no complaint type?
- Because a list containing harassment, discrimination, bullying or safety is a protected-class taxonomy about named people kept in a browser. The log records that something was raised, neutrally, in as few words as you can manage.
- Can I record the outcome as substantiated?
- No, and there is no option for it. A row is open or it is closed, and closing one means you are finished with it. “Substantiated” is a verdict about a colleague, and a free tool must not hand somebody the button that writes it.
- Where does the investigation go?
- Not here. This is a log, not an investigation file: it records that something was raised and dealt with, and points at where the real file is kept. Statements, findings and conclusions belong in your confidential records.
- What should the title say?
- As little as will let you find the row again. “Concern raised about scheduling on nights” rather than anything about the people involved.
