No severity score and no complaint taxonomy
The register assigns no severity, offers no drop-down of complaint types and makes no determination that anything is reportable to anybody. A classification scheme shipped by a product that has never seen your business ends up shaping what people record rather than describing it, and a severity rating is a judgment somebody should make in words, on the row, where it can be argued with.
A label, not a contact record
The complainant field is whatever lets you find the complaint again — an account number, an order reference, or just “a customer”. There is no field for their address, phone or email, none for injuries or medical detail, and none for anything touching a protected characteristic. No decision about a complaint should turn on any of those, and the register is built so it cannot.

- Can I record the customer’s contact details?
- No. There is no field for contact details, and there should not be one here — a label that lets you find the complaint is enough. Their details live wherever your customer records live.
- What does closed mean?
- It means you are satisfied it is settled. It is not a finding, and nothing here takes a view on who was right.
