Mobility and assistance needs are health information about a named person. It stays on this device and is never sent anywhere — but the printed sheet does leave the screen, so treat it the way you treat a care plan.
The clock starts the day somebody moves in
Two of the verified rules here are deadlines that begin at admission, which is the busiest hour in a building and the least likely one to be counting. Illinois allows ten days to orient a resident to the emergency and evacuation plans, signed. North Dakota allows five for a fire drill walk-through. Enter the admission date and this counts from it, and says overdue before a surveyor does.
One sheet each, on purpose
This is the only document on the site printed per person rather than per building. Massachusetts requires every resident to be given a copy of their own instructions, and a single roster listing everybody’s mobility and assistance needs cannot be handed to anyone — giving it to one resident discloses another resident’s health information.
For the same reason nothing typed here leaves your browser, and the shared building record stores only that an orientation happened and on what date. A timeline is for dates, not for diagnoses.
The staff half of the same requirement
States that require residents to be oriented generally also require staff to be trained, and a survey asks for both. The staff training record is the other document, the fire drill log is the drill these sheets prepare people for, and all three tick items on the survey readiness sheet without anything being entered twice.

Other states are absent because we have not verified a resident-orientation rule for them, not because none exists. The sheet works anywhere; it just will not assert a deadline we cannot cite.
