There is no published check list, no tolerance, no cover dimension and no pass mark anywhere in this tool. Those are in your project specification, and the decision to place concrete belongs to whoever administers it and, where one is involved, the design professional.

Before and during

What was actually placed goes on the concrete pour record. If the walk finds work that does not match the drawings, that is the nonconformance log, and an inspection you have to book is the inspection request log.

What checks should be on the list?
The ones your job needs, typed by you. A pre-filled list from a website becomes a list somebody believes is complete, and a slab is an expensive place to find out it was not. Formwork, reinforcement, embeds, sleeves, block-outs, cover, access and clean-out are where most people start.
How does this relate to the pour record?
This is before, that is during. The pre-pour checklist is what you looked at; the concrete pour record is what arrived — ticket by ticket, with the quantities and measurements taken on each load. Both file against the same job.
Can I keep separate lists for separate pours?
Yes. The pour or area goes in the reference on each item, so one job can carry checks for the level 2 slab and the north footings at the same time without them running together.
