The building

The distinction above is the one this tool exists to get right. Most eyewash checklists print ANSI's numbers under an OSHA heading, which tells somebody they are breaking a federal regulation when they are not — and tells somebody else they are finished when their own state plan or accrediting body asks for more. The log records what you found; which of the two applies to your workplace is not something this tool can determine.

Where the reference figures come from

Regulation 29 CFR 1910.151(c). Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. The regulation says no more than that: it sets no flushing time, no flow rate, no water temperature and no testing interval. Consensus standard ANSI/ISEA Z358.1, 2014. The consensus standard that DOES describe flushing time, flow, temperature, weekly activation and annual inspection. OSHA has not adopted it and does not enforce it as a regulation — OSHA's own letters of interpretation say it is referred to as guidance on what a suitable facility looks like. Eyewash / shower — weekly activation & flush of dead-leg water; confirm flushing fluid. Run each eyewash and drench shower long enough to flush out the stale water sitting in the branch line, and confirm both eye streams come up evenly. That standing water grows bacteria, which is the whole point of the weekly flush. Why the checklist is worded the way it is

The eight questions are things somebody standing at the station can honestly answer: can you reach it, does it turn on and stay on without being held, does water come out of both heads, does it run clear once the dead leg is flushed. Each one is Pass, Needs attention or N/A — and N/A is a real answer, because a self-contained unit has no plumbing to flush and a unit over a floor drain has no run-off problem to report.
