It reads nothing

There is no clause analysis, no comparison to a standard, no flagging and no summary. A product that appeared to be reviewing your contracts would be relied on for exactly that, by somebody who then did not read a clause. What this gives you is the administrative picture: what is out, with whom, and how long it has been there.

Cleared is not signed

The stage list ends with the reviewer finishing, not with a contract being in force. Whether something was agreed, executed or commenced is a different fact recorded somewhere else, and a tracker that conflated the two would be a tracker somebody made a commercial decision from.

Where it sits

The file is in the matter tracker, dates you were given in the date list, and material being produced in the document request tracker. On the buying side, quotes are compared in bid comparison and orders are logged in the purchase order log.

Does an entry here mean a contract is approved or in force?
No, and that is worth saying plainly. Cleared means the reviewer has finished looking at it. It says nothing about whether the contract was agreed, signed, executed or is in force — those are events that happen elsewhere and are recorded wherever you record them.
Why Comments out as a separate state?
Because it is where documents actually sit. A review that has gone back with comments is not finished and is not with you: it is with whoever has to respond. Colouring it as returned rather than as open work stops your list reading as a backlog you are ignoring.
Can I keep the contract itself here?
