This is not a permit, and it must never be used as one
An entry permit authorizes entry. It is issued by somebody your own program entitles to issue it, after the hazards have been assessed, the space isolated, the atmosphere tested against acceptance criteria your program sets, and rescue arranged. It is the document that makes an entry permissible.
This sheet is none of that. It is one thing: the attendant’s running note of who went in and who came back out, with the times. It carries no hazard assessment, no atmospheric testing or acceptance criteria, no ventilation or isolation procedure, no rescue plan, no communication plan, no “valid until” and no authorization line — and there is no field for any of them, so that this can never be mistaken for the permit or filled in instead of one.
Why the log exists at all
Because the attendant’s count is the thing that matters minute to minute. How many people are inside, who they are, when each went in, and whether the number coming out matches. That is written on a clipboard at the hole, and it is worth writing legibly.
